今回は、AGA治療、ザガーロ、フォリックス（ミノキシジル塗り薬）、ミノキシジル頭皮注射、それぞれの効果について解説します。

 

 

 

■ AGA治療に使われる

「ザガーロ」「フォリックス（ミノキシジル塗り薬）」「ミノキシジル頭皮注射」

それぞれの効果について

 

1. ザガーロ（デュタステリド）

ザガーロは、内服薬であり、

体内での「脱毛ホルモン（DHT：ジヒドロテストステロン）」の生成を強力に抑える薬です。

 

AGA（男性型脱毛症）の根本的な原因は、

男性ホルモン（テストステロン）が5αリダクターゼという酵素によってDHTに変換され、

このDHTが毛根に悪影響を与えて、ヘアサイクル（髪の生え変わりの周期）が短縮し、髪が細く短くなることにあります。

 

ザガーロは、この「5αリダクターゼ」という酵素の働きを強力に抑え、

DHTの量を大幅に減少させることで、抜け毛を減らし、髪の成長を守る効果を発揮します。

 

特にザガーロは、

5αリダクターゼの1型と2型の両方を抑制できるため、

以前からあるプロペシア（フィナステリド）よりも広範囲の脱毛に効果的とされています。

 

つまり、

 

抜け毛予防

 

進行抑制

 

現存している毛髪の太さと強さの維持

が主な効果です。

 

2. フォリックス（ミノキシジル外用薬）

フォリックスは、ミノキシジルを主成分とした塗り薬（外用薬）です。

日本で認可されているリアップ（大正製薬）と同じ有効成分ですが、

フォリックスは濃度が5%、10%、12%など複数あり、より高濃度版も存在します。

 

ミノキシジルは、

もともと血管拡張作用を持っており、頭皮の血流を改善させることで、

毛根への栄養供給を増やし、発毛を促進すると考えられています。

 

また、ミノキシジルは

 

毛母細胞の増殖促進

 

成長期毛の延長

にも関与しているとされ、新しい髪を生やす力を持っています。

 

つまり、

 

髪の毛の再生（発毛促進）

 

細い髪を太く育てる

が主な効果です。

 

ただし、外用薬は、頭皮に直接塗るため作用は局所的であり、

全身への影響は基本的に少ないですが、塗布漏れに注意する必要があります。

 

3. ミノキシジル頭皮注射（ミノキシジルメソセラピー）

ミノキシジル頭皮注射とは、

ミノキシジルなどの発毛成分を、針を使って頭皮の深い層に直接注入する治療法です。

 

通常の外用薬では、頭皮のバリア（角質層）があって浸透が限られますが、

注射ならば直接、毛乳頭や毛包周囲にミノキシジルを届けることができ、

より強力かつ即効性の高い発毛刺激が期待できます。

 

このため、

 

より短期間で効果を出したい場合

 

重症のAGAに対して

 

塗り薬の効果が不十分な場合

などに特に用いられます。

 

効果としては、

 

新たな発毛の促進

 

休止期に入っている毛根の活性化

 

髪の太さ・密度の改善

が期待できます。

 

施術間隔は通常、2～4週間ごとに数回行い、その後メンテナンスへ移行します。

 

■ まとめ

ザガーロは、脱毛ホルモンDHTを抑えて、抜け毛を防ぎ、髪を守る薬。

 

フォリックスは、頭皮の血流を促進して、発毛を促す塗り薬。

Understanding AGA Treatment: Comparing Zagallo, Follics (Topical Minoxidil), and Minoxidil Scalp Injections

Hello, I’m Dr. Koji Kato from W Skin Clinic Nagoya.

Today, I’d like to explain the effects and differences between three major treatments used for AGA (Androgenetic Alopecia): Zagallo, Follics (topical minoxidil), and minoxidil scalp injections.

 

1. Zagallo (Dutasteride)

Zagallo is an oral medication that powerfully inhibits the production of dihydrotestosterone (DHT)—the hormone responsible for AGA.

 

AGA occurs when testosterone is converted into DHT by the enzyme 5α-reductase, and this DHT miniaturizes hair follicles, shortening the hair growth cycle and producing thinner, weaker hair.

 

Zagallo blocks both Type 1 and Type 2 forms of 5α-reductase, making it more effective than finasteride (Propecia), which only blocks Type 2.

 

Primary effects of Zagallo:

 

Prevents further hair loss

 

Slows the progression of AGA

 

Helps maintain the thickness and strength of existing hair

 

2. Follics (Topical Minoxidil)

Follics is a topical solution containing minoxidil, similar to Japan’s over-the-counter product Rogaine (RiUP), but available in higher concentrations (e.g., 5%, 10%, 12%).

 

Minoxidil improves scalp blood flow, delivering more nutrients to the hair roots. It also stimulates hair follicle cells and extends the anagen (growth) phase of the hair cycle.

 

Primary effects of topical minoxidil:

 

Stimulates new hair growth

 

Thickens existing thin hair

 

While effective, it acts locally and must be applied consistently to clean scalp areas. Care must be taken to avoid contact with unintended areas.

 

3. Minoxidil Scalp Injections (Mesotherapy)

This treatment involves injecting minoxidil and other growth factors directly into the deeper layers of the scalp, bypassing the barrier of the outer skin.

 

Because the medication is delivered straight to the hair follicles, the effect is more direct and faster-acting than with topical application.

Those who want faster results

 

Moderate to severe AGA

 

Patients who have not responded well to topical treatments

 

Expected benefits:

 

Promotes new hair growth
